Firefox: 'Warn me when closing multiple tabs' does not work

Even when I have "Warn me when closing multiple tabs" checked under Options, Firefox still quits without providing the warning. This occurs sporadically on Windows XP. Very aggravating when you have many windows open, and you lose everything with an accidental keystroke.

Even worse - there is actually a keyboard shortcut combination to close the entire window (all tabs included!), and it also does not respect the "Warn me when closing multiple tab" setting. I find this keystroke easier to hit accidentally because I often close single tabs with Ctrl+W, but if my finger also hits Shift (right above Ctrl), there goes everything!
